All nominations are free to enter and the closing date is 16 November 2018.
Nominations are open to non-medical employees and volunteers working at any NHS organisation.
Please ensure that you have read the criteria for the category that you are entering an individual/team into. Remember to use evidence, such as quotes, statistics, etc and include as much information as possible into your entry within the 500-word limit. Please complete all sections of the form, entries that are incomplete or exceed the stated word limit may not be accepted.
All completed nominations will be acknowledged online at the time of submission and a confirmation email will be sent containing details of your submission/s. Please check all details of your confirmation email/s and contact us if you have any queries.
The Unsung Hero Award reserves the right to extend the closing deadline or amend award categories accordingly. The organisers reserve the right to publish submissions from the winners and shortlisted entrants for PR & Social Media purposes. Where certain information is sensitive, such as budget figures, or personal details, please mark ‘not for publication' within the text. Winning case studies may be used in post-event materials to promote best practice.
Judges review and score entries remotely, to nominate their desired shortlist. All judges pre-scores are combined and the highest scoring nominations will determine the shortlist. The shortlist will be announced on 7 January 2019 and the winners will be announced at the ceremony event on 1 March 2019.
The judges’ decision is final and no correspondence or discussion will be entered, or feedback given regarding the selection of shortlisted nominations. The winners of the selected categories will be decided by the judging panel, except the Special Mention Award, which will be decided by an audience vote on the night of the award ceremony.

How your information is collected.
The Unsung Hero Awards is the “controller” of this information. This means that we at Unsung Hero Awards decide how your personal information is used and the ways in which it is processed. The online entry form is hosted and provided by third party suppliers Don't Panic Projects and Fluid Digital, on behalf of Unsung Hero Awards.
What information do we collect?
When you complete an application form on our website (nominator, nominee, newsletter subscriber, brochure downloads), we ask you to provide your name, e-mail address, postal address and your telephone number(s). This information is collected and stored on a secure password protected database and is retained so that we can contact you easily.
Other personal information provided by you (name, address etc.) is used to contact you or for the purposes of merchandise/certificate/brochure delivery.
How do we use your information?
Limited personal information provided by nominators (name, email address and contact telephone number) are shared with other assessors and applicants as part of the assessor allocation/judging process. This again is solely for the purpose of conducting our business and service.
We do not sell, trade or rent your personal information to other parties.
Choices you have about your information
Our goal is to give you simple and meaningful choices regarding your information. Should you wish to delete your entry, please notify us via email at unsungheroawards.com and we will remove it from our system within 5 working days.
How long we will keep your information
We keep your information only so long as we need it to provide you with Unsung Hero Awards news, updates and communications in order fulfil our business service for the purposes described in this policy. Should you receive an e-newsletter or other communication from us, you can unsubscribe at any time via the link provided at the bottom of the email.
You have options in relation to the information that we have about you described below. To exercise these options, please contact us at firstname.lastname@example.org
- Access the information we hold about you. We'll usually share this with you within 30 days of you asking us for it.
- Have your information corrected or deleted. You can update your information. If you have problems updating the information or if you would like us to delete it, contact us.
- Object to us processing your information. You can ask us to stop using your information, including when we use your information to send you newsletters or marketing emails and in Social Media. If you opt out of receiving these messages from us, we may still send you direct emails or updates about your account. We only send you marketing material if you've agreed to it, but if you'd rather we don't, you can easily unsubscribe at any time.
- Complain to a regulator if you feel we haven't complied with the Data Protection laws.
