Nominee's story :

Ruth has shown resourcefulness, innovation and outstanding commitment in the development of our trail-blazing holistic demographic questions template on a main EPR system 'SystmOne'. Ruth responds with alacrity and insight to all my requests for extending the scope of questions, or for making adjustments to them, whilst keeping within the confines of NHS 'read codes' (SNOMED), which ensure that data is recorded and attached to the patient's record in a way that can be shared with other SystmOne users. A comprehensive 'accessible information' template is embedded in the demographic template, so we are able to meet the requirements of the Accessible Information Standard. Ruth is always candid and often comes up with ideas which remove barriers and enable us to tailor the process of capturing such precious patient data to meet the needs of our different services. The best example of this is the creation of a questionnaire based on the same questions as in the demographic template, but in a format that is sent by email or text directly to the patient for them to complete outside of their appointment. The beauty of this version, is that when the questionnaire returns to our organisation, the responses revert to their corresponding 'read code' in the patient's electronic health record. Ruth will say she didn't invent this, but to have the knowledge and insight to pursue this option for a particular group of staff pressured by time constraints has been a revelation. Despite having a very demanding home-life, Ruth and her team have rolled out the new demographic template and associated equality questionnaire out-of-hours, so as not to encroach on clinical time. Our patients' responses to the questions will give staff invaluable insight into their 'protected characteristics' and domestic circumstances, helping us to gain greater understanding of individual needs and where people are susceptible to experiencing health inequalities. Ruth is the person who has anchored this work at the heart of service delivery. I can't thank her enough.
